I understand that eyelash extension application requires the use of adhesive, tape, cleansing

products, and tools near my eye area. I acknowledge that I have informed my lash artist of

any allergies, sensitivities, medical conditions, medications, or eye conditions that may

affect my service.*

I understand that possible risks may include redness, irritation, allergic reactions, itching,

watery eyes, or discomfort. I understand that results may vary and that Blinx Lashes is not

responsible for reactions caused by undisclosed conditions or failure to follow aftercare

instructions.*

I agree to follow all lash aftercare instructions provided by my lash artist, including

avoiding oil-based products, excessive rubbing, steam, or pulling on lash extensions. I

understand that improper aftercare may affect lash retention.*

I voluntarily consent to receive lash extension services from Blinx Lashes. I release Blinx

Lashes, its owner, employees, and lash artists from liability for any injury, reaction, or

damages that may result from the service provided.*

I give permission for photos/videos of my lash services to be used on social media and

marketing materials.

I understand that deposits may be non-refundable and that cancellations or no-shows may

be subject to fees according to Blinx Lashes booking policies.*
